Registration Forms Required:
For Football: The following forms are to be completed and submitted at equipment/uniform fittings:
1. BIRTH CERTIFICATE: Original Birth Certificate (with raised seal) AND a Photo Copy of the birth certificate
Pop Warner requires that a member of our governing league Bux-Mont, verify all participant birth certificates. QFCA staff must have the original or a certified copy (with raised seal) to look at--photocopies will NOT be allowed. Once they verify the authenticity of the birth certificate, they will stamp the copy which will be added to the Team Book. After Bux-Mont has stamped the copy, the original will be returned to you. Once your athlete's birth certificate is on file, you will not have to provide it again for future seasons
2. Dr Physical Form - the medical clearance forms are due prior to the first day of practice and must be completed on the 2026 Pop Warner Physical Fitness & Medical History Form or pages 5&6 of the PIAA Form
3. Report Card for current school year: Report Cards are required to receive equipment (2 copies).
1 copy must be the Final Report Card for the 2025/2026 School Year
